DAI (also referred to as traumatic axonal injury, or TAI) is a type of traumatic brain injury (TBI) resulting from accelerating and decelerating motion, with a rotational component, that leads to shearing of the white matter tracts of the brain Clinical presentation ranges from clinically insignificant findings to a comatose state Severe traumatic brain injuries (TBI), commonly due to motor vehicle accidents may cause death and long-term disability especially when the acceleration-deceleration force on the brain is massive. This may cause shearing of the axonal connections within the cerebral cortex and brainstem in a process referred to as diffuse axonal injury (DAI). Diffuse axonal injury occurs when the brain quickly moves inside the skull as a result of a traumatic injury, like a car accident. As the brain repeatedly hits against the inside of the skull from the force of the injury, the long connecting fibers in the brain, known as axons, can tear and potentially cause severe damage.

However, most patients with DAI are identified to be severe and commonly have a GCS of less than 8. Diffuse axonal brain injury (DAI) is a form of extensive lesions found in the white matter of the brain. This type of brain injury occurs to about half of all cases of brain injury. Thus, it is known as the most common and devastating type of brain injury. It causes unconsciousness and patients may end up in a vegetative state.

The aim of this article is to describe the epidemiologic, clinical, and radiologic features of posttraumatic DAI and to study its prognosis impact on mortality and outcome according to Glasgow Outcome Scale. grade 3: focal lesions in both the corpus callosum and dorsolateral quadrant of the rostral brainstem, in addition to diffuse axonal damage; The features of diffuse axonal damage vary by time: axonal bulbs are present within days, microglia clusters and astrocytosis develop after weeks, and Wallerian degeneration of the white matter tracts sets in after months.

Som "diffusa" i namnet antyder, kan en diffus axonal skada täcka ett stort område i hjärnan, i motsats till en samlingspunkt skada, där skadan är koncentrerad till en region. Diffuse axonal injury (DAI) is one of the most deleterious forms of traumatic brain injuries (TBI), which are thought to occur as a result of the application of rapid rotational loads onto the head. In the literature, injury thresholds have been proposed both at the whole brain level and at the tissue level. Clinical outcome after traumatic diffuse axonal injury (DAI) is difficult to predict.
However, most patients with DAI are identified to be severe and commonly have a GCS of less than 8.
